Under the direction of the Care Manager, the Care Coordinator supports the company’s goals by providing an excellent customer experience to both Health Service Workers employed by us and Clients receiving services from us. The Care Coordinator strives to achieve process efficiencies, exceed customer expectations and offer a seamless experience to our Clients and Health Service Workers by ensuring that all policies and procedures are being properly followed at each case.
Key Responsibilities

To visit at least 8 clients per week if Part Time or at least 15 clients per week if Full Time

Assist in caregiving for our clients

Assist Care Manager with duties such as field visits, addressing client concerns, introducing Health Service Workers to new clients, and visit clients where concerns have been raised

Can complete necessary responsibilities set forth by the Care Manager in their absence

Be on call at least once every 4 weeks

Assist in scheduling on an as needed basis

Positive and friendly voice tone when answering phone calls and addressing concerns

Home Care Pulse Monthly reviews of Clients must be over 90% satisfactory

Fill in on cases when necessary
Skills and Qualifications

C.N.A. certification required

Strong interpersonal and communication skills both orally and written

Strong and efficient problem-solver and multi-tasker

Proficient knowledge of home care industry software use

Knowledge in Caregiving or Home Care Industry preferred
