Primary Care Physician

SHHC delivers holistic care to help every member of the family stay healthy and active from infancy through adulthood. 

Main Duties & Responsibilities
  • Improve health and well-being in diverse underserved communities.
  • Provide ongoing comprehensive primary care to patients of all ages alongside an interdisciplinary health care team.
  • Become a member of Advocate Physician Partners and participate in Medicare Shared Savings Program (MSSP) – CMS’ value based care program focused on promoting health equity and addressing health care disparities for underserved communities. 
Requirements
  • MD or DO from accredited institution, licensed physician in Illinois.
  • Federal DEA license, Illinois Physician’s license.
  • Board Eligible or Certified in Family Medicine or Internal Medicine.
  • Interest in and comfort with working with diverse patient population. 

 

Job Type: Part-time Independent Contractor 

Schedule: 2-3 days per week (6.5 hour shifts)

About the SIDNEY HILLMAN HEALTH CENTRE of Chicago:

History:

Sidney Hillman Health Centre (SHHC) was named to honor a union organizer and labor advisor to Franklin D. Roosevelt. Sidney Hillman advocated labor union activity to assure provision for the social and health needs of working people and their families. The health center was opened as a voluntary association by the Amalgamated Clothing and Textile Workers Union in 1956 (now Workers United,) to provide affordable comprehensive health care to union members, their dependents and retirees. Now also serving the general diverse community.

Who we are: 

SHHC is a multispecialty health center. We offer medical, some specialty and dental services. Our health care philosophy emphasizes helping individuals maintain and improve health through the use of recognized preventive services. Our physicians are all Board Certified specialists skilled in handling a variety of acute and chronic health conditions. All of SHHC staff work to provide an effective health care home for the patients who use us for primary medical care, so our staff actively arrange and coordinate care even for services obtained at other institutions. We believe high quality health care requires up to date medical information and respectful and coordinated service.

We are close partners with Chicago Midwest Regional Joint Board of Workers United.
